Apply for Services

If you are moving into a home inside City limits of Medical Lake and need to establish your utility account,  these are the steps to do so:

• Determine when you will be moving in and/ or taking possession of the property.
• Determine if the property's water service is on or off.
• Gather the personal information of all applicants (i.e. drivers license number, employment information, and personal references)
• Complete and submit our New Account Form.

Click here to download a New Account Form. This forms can also be requested in person at  City Hall, by phone, or email. Signed and completed forms can be either mailed, dropped off or faxed to City Hall.
